Therapeutics

Use

  • Sole Use: Uncommon, usually used as part of polypharmacy cardiology protocols.

  • Multimodal Use: Amiloride may be deployed alongside other agents such as ACE inhibitors,  pimobendan, digoxin, and other diuretics such as furosemide, spironolactone or hydrochlorothiazide.

  • Treatment Goals: Potassium-sparing diuresis.

  • Treatment Endpoints: Poorly defined. Reduction of the effects of oedema where patients are hypokalaemic or likely to become hypokalaemic.

Efficacy

  • Efficacy Profile: Efficacy remains unproven.

Adverse Effects and Contraindications

  • Adverse Effects Profile: Amiloride is likely to be well tolerated in dogs, but there is currently inadequate data to confirm this. Except for hyperkalemia, significant side effects are infrequent.

  • Contraindications: Hypoadrenocorticism, hyperkalaemia or hyponatraemia, anuria, acute kidney injury, or significant renal impairment. Do not administer in conjunction with NSAIDs to dogs with renal insufficiency.

Monitoring

  • Monitoring: Electrolyte monitoring is advisable. Monitor renal output and function.

Reproduction

  1. Pregnancy: Unknown; Amiloride is not recommended for use during pregnancy or lactation.

  2. Lactation: Unknown; Amiloride is not recommended for use during lactation.

  3. Male Fertility: Unknown; Use with caution.

  4. Female Fertility: Unknown; Use with caution.
